Manage My Vessel's Form Builder allows users to build their paper forms into electronic forms so their crew on the vessel may use electronically.
Building Forms
Accessing the Form Builder
Click the Modules menu.
Select Forms.
On the blue ribbon, click Administration.
Select Form Templates.
Choose the folder where your template will be organized.
On the top-right corner, click the three dots.
Select New Form Template.
Setting Up Form Properties
Enter the form title.
Add a description.
Enable or disable:
Open or close status
Correspondence (if needed)
Attachments
Digital signature
Requirement for users to change the form title before submission
Add any necessary header and footer information.
Opening the Form Builder
Under the Content section, click the hyperlink to open the Form Builder.
The first section shown is PDF Orientation.
Choose:
Portrait or
Landscape
For this tutorial, select Portrait orientation.
Creating the Form Layout
Drag a table section into the form.
Choose your table layout.
Drag and drop a text block into the table.
Text blocks are used to add static text.
You can:
Bold text
Adjust font size
Change font family
Continue adding new tables based on how you want your form to appear, and drag the required tools from the left-hand panel into the form.
Duplicating Rows and Tables
To duplicate a row:
Hover over the row
Right-click
Select Duplicate Row
This can also be done for columns and tables.
Edit duplicated text by clicking and typing the new content.
Publishing the Form
Once complete, click Submit.
The form code will be inserted into the content section.
For future edits:
Use the Form Builder link
Avoid editing XML directly unless experienced
Choose whether the form is visible to:
Specific vessels or All Accounts
Click Publish.
Enter any necessary comments.
Click Publish again.
