You may require an electronic form to be completed as part of a vessel’s mandatory document or a specific crew document.
Click on the Modules menu and select Documents.
In the top blue ribbon, click the Administration menu.
Select either Crew Mandatory Documents or Vessel Mandatory Documents, depending on which type of mandatory document you want to add a form to.
Click on the three dots next to the template and select Edit.
Locate the Form Template dropdown list.
The dropdown will display only the electronic forms that are currently active in your system.
Once done, click Submit.
