Creating Electronic Forms Manually

Permission-based access: What you can see and do in MMV depends on how your management company has set up your account. If a feature described here isn't visible in your account, ask your admin.

  1. Click on the Modules menu, then select Forms.
  2. On the blue ribbon, click Administration, then select Form Templates.
  3. In the top-right corner, click the three dots (⋮) and select New Form Template.
  4. Enter a title, choose a folder, and provide a description for the form.
  5. Enable or disable the following options as needed:
    • Open/Close Status -- Sets whether the form is active (open) or inactive (closed).
    • Correspondence -- Enables communication or comments related to the form.
    • Attachments -- Allows crew members to upload files when completing the form.
    • Digital Signature -- Lets crew members sign the form electronically.
    • Require User to Change the Form Title Before Submission -- Ensures users personalize or update the form title before submitting it.
  6. Add any necessary header or footer information.
  7. Under the Content section, click the Use Form Builder.
  8. On the toolbox on the left-hand side, start by dragging the Table and dropping it onto the "Drop Table Here" field.

    You will then be asked to enter the number of rows and columns you want to create.

    For example, to add a header:\

    Create a table with 1 row and 1 column.

  9. Drag a Text Block into the table.
  10. Enter the text and format (bold, font size, alignment, etc.) located in the left-hand side.
  11. Add a new table with 2 rows and 2 columns.

    Drag and drop Text Blocks or Text Boxes in each cell as needed.

    • A Text Block displays text.
    • A Text Box allows users to type in text electronically.
  12. On the left-hand side of the screen, you can continue dragging and dropping your desired elements to build the form. Below is a description of each available tool:
    • All User Picker -- Displays a dropdown list of all users associated with the vessel.
    • Checkbox -- Allows users to select or tick one or more options.
    • Crew Picker -- Displays a list of active crew members assigned to the vessel.
    • Crew Position Picker -- Let users select from pre-defined crew positions.
    • Date Picker -- Enables users to select a date from a calendar view.
    • Fleet User Picker -- Provides a dropdown list of all fleet users.
    • Hyperlink -- Allows you to add a clickable link within the form.
    • Image -- Lets you attach or display an image in the form.
    • Multi Fleet User Selector -- Allows selection of multiple fleet users at once.
    • Multi Crew Selector -- Enables selection of multiple crew members simultaneously.
    • Multi Option -- Allows to select multiple answer for users to select from.
    • Number Selector -- Provide a dropdown with numeric values only.
    • Options -- Displays a dropdown list where the user can select only one answer.
    • Signature Pad -- Provides a field where users can sign electronically.
    • Time Picker -- Allows users to select a specific time.
    • Vessel Picker -- Displays a list of vessels from which the user can choose.

Edit Cell Properties

  1. This section allows you to customize the appearance and layout of your table cells.\

    You can:

    • Adjust the row and column span to merge or expand cells.
    • Set the vertical and horizontal alignment of the content within the cell.
    • Choose the font style and adjust the cell width as needed.
    • Change the background color of the cell to match your desired format or design.

Edit Row Properties

  1. This section allows you to customize the appearance and layout of the row.\

    You can:

    • Set the vertical and horizontal alignment of the content within the row.
    • Change the background color to match your design preference.
    • Enter the desired row height.

Edit Table Properties

  1. This section allows you to customize the appearance and layout of your table.\

    You can:

    • Choose whether to show or hide the grid lines.
    • Set the table width, with a maximum of 1100 pixels.
    • Add top or bottom margins (e.g., 5 or 10 pixels) to create space between this table and other sections.

Edit Form Properties

  1. This section allows you to customize the orientation and width of the form.\

    You can choose between portrait or landscape layout and set the desired form width to ensure proper formatting when displayed or printed.

    To perform actions such as deleting, copying, inserting, duplicating, or moving a row, column, or table, hover your cursor over the section, right-click, and choose the appropriate option from the menu.

    When creating a form, always click Save Properties to make sure all changes are properly applied and saved.

    Once done building the form, click Submit.

    Select which account this form will be visible.

    If you are satisfied and ready to publish the form, click Publish. If you would like to save the form for review before publishing, click Save as Draft.
