This section of the guide will show you how to define fields within a form template so that you can use them for form reports and triggers & automations.

From the Fleet Account page go to ‘Manuals, Forms & Checklists’ – ‘Manage Form Templates and Profiles’
Find the form you want to modify and click the ‘Edit Properties’ button.
Scroll down the page and click ‘Edit’ just below the footer field.
This will load a page with the form template on the right and a properties window on the left hand side. To define a field click on the field in the form, on the right hand side, which will then bring up a display name field in the properties window. Enter the display name and then click on the next field to define.
Pro Tip: Be descriptive when entering the display name as this is what will be used when creating form reports and triggers.
Once you have defined all the fields in the form click Submit at the top of the page and then click ‘Submit and Check In’.
